You do not need to register your child if they are returning Ashford student.

NO CHILD WILL BE PRE-ENROLLED WITHOUT UP-TO-DATE MEDICAL REQUIREMENTS AND A BIRTH CERTIFICATE.

PLEASE BRING THE FOLLOWING ITEMS( WITH COPIES) FOR REGISTRATION.

- Proof of Birth Date (must be four by September 1, 2009 to enter Pre-Kindergarten; must be five by September 1, 2009, to enter Kindergarten, must be six to enter first grade.

- Proof of Immunizations:
Dipteria and Tetanus (Booster given since fourth birthday)
Polio (Booster given since fourth birthday)
Rubella Vaccine (Three Day Measles-German Measles)
Rubeola Vaccine (Red Measles) on or after first birthday
Mumps (Anytime) - required through age 16
Haemophilus Influenza Type B (HIB) Ages 18 months-four years
Hepatitis A - Two to fours years of age
Hepatitis B Vaccine - Three doses for children born after 9/2/92
Varicella (Chicken Pox) born or after September 2, 1994


- Proof of Residency in the Westside Zone - utility/phone bill with the parent's name and address.

- Transfer or report card from previous school, if applicable.

- Child Social Security Card

- Parent Driver License or Identification Card
